FMLA Reporting Changes
Effective January 1, 2008
Effective January 1, 2008 the following changes will go into effect for reporting FMLA:
1. Time will no longer be a “rolling calendar”. It will be based on January – December year.
2. Employees will be given their entitlement at the beginning of the year based on the previous 12 months of eligible hours worked.
3. Upon your return to work, you MUST report your need for FMLA to your Supervisor.
4. Your Supervisor will be responsible for submitting FMLA requests via the online intake process. Your Supervisor will provide you with bar-coded medical certification forms specific to you.
5. Medical Certifications may also be obtained before a request for leave is submitted.
6. You will have the same 15 day time allotment to have the papers filled out by the physician.
7. ONLY the PHYSICIANS office can fax papers to the FMLA reporting center, or you can mail them yourself.
8. The FMLA reporting center will go by the date of the postmark, not the day they receive them to determine 15 day guideline eligibility.
9. There will now be a “self-service” FMLA status site that will provide an 18 month history of claims processed by at&t FMLA Operations (status for closed claims managed by Sedgewick CMS will not be available). Access is provided to employees, supervisors, SPOC’s and HR managers on the status of FMLA request, which includes all absence detail, medical certification receipt and due dates.
10.There will also be a Self –Service FMLA Determination Letter Retrieval should employees or supervisors misplace their determination letters. Copies can be retrieved and printed via the FMLA status site.
11.Employees are encouraged to elect to receive the FMLA determination letters via email at work, home or both.
